Are you using any speedhacks(Config -> Emulation Settings -> Speedhacks tab)? If so, try turning them off(remove preset in lower left corner if necessary).

nope all turned off. It's odd a level will start for a few seconds very slowly. Then quickly ramp up to where it's playing at about the normal speed + 1/4

My version of THPS3 is NTSC USA RIP and works even with MTVU hack.
THPS4 and newer won't load with MTVU.

actually looking at that brief snippet although hard to tell as it's a short clip it also looks like yours is running to fast as well compared to playing on an actual PS2.

No that's full speed 60FPS.
