I think this might be caused by PCSX2 not detecting the "key up" stage ... the alt tab is treated by Windows, not by it. Hence hitting alt again the key release is registered.
I mean, the problem is the program might be seeing the Alt key is still pressed, till pressed again and then released.
Edit: a possible solution is checking the actual key status and forcing release case the physical key is not pressed. Were not for possible problems to users due to shortcut keys non functional it would be minor issue, but how it is it is really confusing.
