I have the same problem on my PC, no manager/SOUND problem.. I can't get to main menu screen, game hangs with all Configure->CPU flags off. Also, if Configure->CPU VU options on, game breaks with memory access error after all intros.
My config: Core2Duo 6750, 8800GT, 2GB DDR.

It seems to be playable now with pcsx2 svn 1040 and gsdx svn 1006!
Speed is great with hack enabled. Only scene 3d have texture problems but it's not the most important in Guitar hero games.
Guitar hero metallica works but the speed is a big problem here to play.
