I knew it would be a mess. Start by turning the EE cyclerate hack down a bit, to 2 maybe. Also, cycle stealing on 1, not 3, you're asking for problems. Also turn off all manual game fixes selected. Unless there's a game that requires them, you shouldn't run them with the others.
I don't know if your framerate in the game suffers, but if it does consider turning the resolution down. It should still look really crisp at x4.

Next up, your GSdx. Turn off the following:
Shadeboost (i doubt you're using any custom settings)
FXAA (do you really need pseudo anti-aliasing?)
FX shader (you're probably not even using a shader, so no point in having it)
Edge anti-aliasing (at x6 scaling you shouldn't even need this)

Set extra rendering threads to 3. You don't have 11 cores, only 4 Tongue2
Turn off ALL GSdx hacks but keep the nVidia one.

Clearly I did not know what i was doing, the game seems to be fine now, thank you! I kept one hack on for better textures at cut scenes, but that one i have tried with and without, and the differences is big.

Edge anti aliasing is for software mode only. It's an actual function of the PS2, and there is no reason not to enable it. It does not affect HW mode at all.

Hey guys, sorry to necro this god-old thread, but I've recently had this problem with newer pcsx2 versions by default, changing nothing but the internal rendering resolution.
Here's what the game looks like:

The fix is the half-pixel offset.
First turn on manual renderer fixes in the rendering tab, then go to the upscaling fixes tab.
Turn off half-pixel offset. Not sure why this was an automatically applied fix.

Also you may have to enable auto-flush in the hardware fixes tab, which was an automatically applied fix that is actually needed, which may be disabled by turning on manual fixes.
