Have you tried doing controller stuff in safe mode?

It seems to me like a DLL got fragged along the way somewhere. If you could isolate it and replace it, you might stand a chance of fixing this without a reinstall.

When PCSX2 crashes, do you get the crash dialog box?

If so, push the more info button and read that stuff. It should tell you the faulting DLL. If you are lucky it will be the actual one. It might just say lilypad though.
