This hasn't been asked yet: is the CRC of your game (you can see it in the PCSX2 console when booting the game) the same as the CRC of the pnach file?

They need to be the same, and it happened to me more than once that I had a different CRC (translation patches and undubs can cause this, for example).

If they aren't the same, try renaming the pnach file to the CRC you have.

Yes...looks like the game saves the save is a way that is readable only on the version used to create it

And at least , is there a language translation for the game?

If the game dn't have a menu from where to choose the language,this is your only choice(and if that doesn't work,then the game don't have that language)
pcsx2=>CDVD=>No Disk=>System=>Boot CDVD=>System Configuration=>Language=>Italian=>press X then O=>restart pcsx2=>CDVD=>ISO=>run the game with Boot CDVD(full)

Even with the PAL game the game is still in English ... There is no way you can extract a language file and put it in another game right ?
