Did you resample the video to 720p? Although youtube should had done it automatically, and I don't know if there's a way to upload pre-encoded videos withing the restrictions, but if it's possible, it's better like that Tongue2.

And I don't know if Youtube supports more than 720p. Does it support 1080p?
Also make sure you click on "High Quality". Only the more newer videos on youtube have it.

What format is the video in, if it's not in the right format, then it's destined to not work.
System Specs:

AMD Athlon™ 64 X2 Dual Core Processor 4200+ @ 2.20Ghz | 1 GB RAM | NVIDIA GeForce 6150 LE 512MB | Windows Vista Home Premium SP1 32bit

type &fmt=18 at the end of the url.
then you should get HD, if available.

Main Rig: i7-3770k @4.5ghz | 16GB DDR3 | Nvidia GTX 980 TI | Win 10 X64
Laptop: Alienware 17 | i7-4710MQ | 16GB DDR3 | Nvidia GTX880M | Win 10 X64

Youtube supports a max resolution of 1280x720 (Any higher and it will still downscale it to 720p) and if you are uploading in 720p you need a minimum bitrate of around 3500kbps.

If you want to encode a video to HD, choose a resolution of 1280x720, and a bitrate of about 4000, and use .MP4 with the H.264 codec.

Also, &fmt=18 = HQ (480x360), High Quality, not HD (1280x720), high definition, to play a video uploaded in HD other than the 'Watch in HD' option, it is &fmt=22.
