There's no vtlb miss error in that emulog, you also didn't seem to use other than the ISO in that log.

Sigh... post your emulog.txt after you get the error you are talking about while trying to run from the Original disc without savestates at all...

But whetever your error seems to be a bad ISO ...

it simply looks like a bad iso.
re-dump it from your DVD
