It still shows "512x448" in the GSdx plugin settings?
Or are you talking about on the titlebar of the actual game's window?

You can ignore that. That's just what's native, not active.
If the setting is changed in GSdx, your internal resolution is increased.
Try this:
When playing a game at that custom res, go into the GSdx settings and simply check Native. After you apply that (click "OK"), you should see a noticable difference in the way the game looks.
