no, there is nothing in pcsx2/patches directory - where i can get patches for this game?

no, check if patches are disabled in the "misc" menu of pcsx2, sometimes patches that were meant for older pcsx2 versions might cause such problems.

well, afaik these tlb miss errors can be just about everything

do you run the game from disc or hdd?

from hdd

do you also get this error with the disc in your optical drive and gigahertz or peops as your cdvd plugin? if not, then the iso image is faulty.
