Probably your GSDX settings, you've pretty much turned everything on.
Turn off Anisotropic filtering and MSAA, use Native x4 or x5 instead of custom resolution.
Oh and you're using an old version of GSDX, make sure you have selected a new one.

I am using the version of gsdx that came with 1.4. I have turned down the native resolution to native. I have turned off Ani filtering and MSAA all to no effect.
Edit: I did this all one at a time. slowly turning everything off and on again.

Select OpenGL (Hardware) as renderer and turn CRC Hack Level down to Partial.
And just to be sure, turn off the MTVU speedhack.

Looks like switching to pcsx2 version 1.2.1 fixed everything. well, that works, I suppose.

openGL shouldn't crash on that setup? I was running on a very similar build with a 970 for a while.
It could be worth checking a new Git build
