i don't think its a bug report anyways. It just a GSDX Question. I guess your GPU is struggling.
Simple isn't it.
Lower D3D Resolution : Lower Quality
and
Higher D3D Quality : Higher Quality = Low FPS if GPU can't handle.
Anyways, Change Windows Power Plan to High Performance

2x same but tryed custom and i found a good 1 not the best but there not as much squared images 1025*640
ty if u find any other sulotion to totaly fix dont be shy ^^

Also, you have optimus. Make sure PCSX2 is running on the nvidia GPU instead intel HD.
create a new profile for the pcsx2.exe in the nvidia control panel and chose the Nvidia GPU.
btw: native is exactly the original PS2 resolution, so no wonder why it looks blocky
