FFX Crashes when enabling patches

Greetings everyone.

I am having a problem with enabling patches with my game. FFX will run without any difficulties, ISO or CD it run great. As soon as I enable patches, reset, and start the game PCSX2 will crash. I have tried to look for a solution on some of the older threads but did not find anything too relevant. The patch that I am using is the default that came with the emulator. I have some information available for you to look at. I have tried to make one of my own using the CodeBreakers method but met with the same conclusion.

That is information I do not know. This and a series of other patch files were included with the emulator from the PCSX website. The one that I created myself is shown below. There is not much to it, I try not to cheat beyond reason, I just wanted a few basic items.

The original decrypted code for the first patch looked like 1031A1B4, I changed the 1 to a 0 as instructed in the documentation I have read.

I replaced the original patch file with my own and I still come across the problem. I have not tested patches with any other game because FFX is the only one I have a desire to play. Any suggestions is greatly appreciated. The error being produced in the console log is below.

Always try with a single code and go on one by one. Putting all the codes together will only result is a crash.

I removed the patches for turbo ether and phoenix down and just left the x potions. I met with the same results, my game crashed when it was enabled. It's too bad there isn't a cheat plugin like the PEC emulation cheater for epsxe. Below is the section of the log with the error.
