1st... do your CPU has SSE3? If not switch to SSE2 graphic plugin.
2nd... make sure you use DX9 mode as i see you have XP installed
3rd... delete all ini files in INIS folder. Then reconfigure all again.

LAST... most important. Edit GSdx-sse2.ini to look like this:

[Settings]
ModeWidth=0
ModeHeight=0
ModeRefreshRate=0
Renderer=3
PixelShaderVersion2=-65024
Interlace=0
AspectRatio=1
filter=1
tvout=0
nloophack=1
vsync=0
logz=1
fba=0
resx=1024
resy=1024
swthreads=1
nativeres=0

Just tested... and it worked.

P.S. Do not use save snapshots to resume your play. If you had missing polys there, you will have them again no matter what settings you put. Remember that.

I just found the reason for missing geometry, which is not related to any fixings in graphic plugins.

NEVER start the game with Run --> Execute.

ALWAYS start the game with File --> Run CD/DVD
