Tested a few games, its in all the games.
I don't understand how to resolve this, nothing works, settings, display rates, ulmb..
Dunno, but many games hare almost unplayable.
I couldnt test your codes because I aint got no KH2 final mix ntsc.

Tested on my tv (plasma panasonic P50ST 60) :
- Ghosting still here in all display rates (60,59,25,24..).
- Tested on 60hz with intelligent frame creation set on maximum, its PERFECT. My god, its beautifull, I can see every details while I move, the imput lag is serious but its still playable on that TV. Without max frame creation there is still some ghosting.

I don't know what to conclude... Maybe the ghosting are here only because the PS2 framerates are very low ? And still I see no way to resolve the issue on my pc monitor.
I'll try to plug a 60hz pc lcd just to see, I think the people dont see ghosting but everyone have it, with a mid range 60hz maybe it just seems normal... I don't know, just wondering.

Still not seeing this issue on either 60Hz Samsung Syncmaster monitor or 100Hz TV (Philips 40PFL5605H).

It has to be really specific this issue. Either it has to do with the TV/monitor, or something else is going on that causes it for you.

There is lot of topics on this on internet, im not the only one.
I have it on a dp port 144hz lcd, then on hdmi latest plasma tv, so thats not specific. Only the intelligent frame creation resolve this.. its an artificial solution.
Ive tested suikoden 5, tale of the abyss and KH2, same problems in all games, the less playable is KH2 because its full 3D fast movements.
Maybe some games don't have this problem, I recently made star ocean 3 and nothing shocked me, dunno if I didnt saw it or if the game didnt had it, I dont have the iso anymore, I think ill check someday.

Even if you're not the only one, something specific has to be causing it. Because while it may be happening for more people, at the same time it's also NOT happening for a lot of people. Myself included.

I've played the games you mentioned from start to finish on both this monitor and the 100Hz TV and have not seen this ghosting you're describing.

The fact that you have it with regular 60 Hz refresh rate is even more indication that something specific is causing it. Question is; what though.

Do you have a different monitor/tv around to test it on? (One that runs at 60 Hz max, or possibly 100 or 120 Hz as well)

Yes my 144hz lcd can run at 120/85/60 hz (I dont remember if it got 100hz..), Ive tested it all, still got the problem.
I got another full 60hz monitor, ill try later.

Tried on a 60hz monitor, there is still a blurry effect but because its an old 60hz the ghosting effect is more difficult to see, I think its an optical illusion but its still a bit too blurry even for a 60hz.
I think somebody who plays on this monitor may not notice the problem if his screen is not smooth in normal situation anyway, so this way I can say its "better".
Its hard to explain with words Laugh
Maybe If I take a photo you could see it.

edit : rechecked, yes we can see some ghosting, its just the flickering effect thats less impressive.
edit 2: my 144hz got a 100hz mode, tested, still the same.

Found a picture of someone who have the same problem, this is what you have with every object moving fast, and its not only a trace, it "blinks", its the ghosting is moving very fast betwen the 2 positions (so fast its hard to see on a 60hz monitor) :

I still don't know why only my plasma TV with intelligent frame creation option at maximum resolve this effect, I don't know much about all this. Tested games with pcsx2 0.9.8, still here.
I'm still searching some ways to resolve this by myself.

edit : its weird because if I take a screenshot, the image is clear, no ghosting. I have no clues -_-
