I didnt make any mistakes when implementing the cheats. I also used the correct CRC and cheat format.

First, burn your disc to an iso to have a better view on your emulog.

Hotswapping might be the problem.. Why do you use the codebreaker iso?

Edit: The crc you try to load cheats from is not included in your emulog.-> You have the wrong pnach file in general or wrong crc put to the filename.
1. Check if you really use the correct version of KH. There are some around. Most prominent example is NTSC-U vs NTSC-J vs PAL. But I think there are also special version as those final mix stuff (not a KH expert so might be wrong)
2. Adapt the pnach-filename to your game crc.
3. Don't use the codebraker-iso. I think this doesn't work with pcsx2 and actually you don't need it due to the pnach-system. It might fix the crc to the codebreaker crc and therefore will not load the correct cheat file. (Once there was a similar problem for savestates. Don't know if this problem is solved for cheats as well)
