It really was that simple, I'm serious. Here are my settings for PCSX2 0.9.7.3113. The settings that aren't shown I left on their defaults.
PCSX2 Settings 01
PCSX2 Settings 02
And this, literally, was all I had to do that made it work.
ISO unmounted
Select 0.9.7's ISO mode

you are supposed to not even mount it in the 1st place so that has nothing to do with it. can a dev confirm it please? cause pcsx2 site has usa version of metal gear solid substance unplayable on pcsx2 0.9.7 (and all before as i can remember) i am not in my pc to test it again but it wasnt working on any settings i tried it (included the ones elektr1k posted)

Weird. If I use "Full" instead of "Fast", the US/NTSC version actually loads. I can confirm this as well; the compatibility list needs to be updated.

I would edit my previous post, but if I did that, it wouldn't show up under "recent" or "new" posts! Here it is: http://www.youtube.com/watch?v=aW9R9k1MN9U

Yeah only iso selector. For other games you can certainly use plugin mode.

yah well can you confirm your settings. I use the cd->iso selector->mgs2s and full boot...PCSX2 just hangs at a black screen.

Why don't you create a new thread.
Try Boot Fast. Make all settings back to normal/default.
