Its also sounds me like throttling of cpu !, (Overheat) ! Make sure your powerplan set to performance !
Core i3 9100f 3.6Ghz
RAM=8GB
nvidia GT 1030
pcsx2 version-1.3.1

MGS2 is a very GPU damanding game, especially in areas with real-time shadows (like the one on the left screenshot).

You GPU is a Low-End one and might be too solw for these areas.

Try these settings for GSDX:
Direct 3D11 Hardware
Native resolution: checked
FXAA/ FX Shader: Off
Allow 8-Bit Textures: On (might help or make things worse, test it out)
