Can anyone tell me what the error code 'vtlb miss : addr 0xC, Mode 0' is?
It seems to appear after the line 'Restart Without Memory Clear Done.'

It's not something in particular,just means that PCSX2 crashed. In order for us to help you,you need to give us way more info,like game you are trying to run,all your PCSX2 settings,plugins,plugin settings and PC specs

It seems to be directly related to the reading of the cd/dvd try dumping the game to an ISO then use the "Linuzappz Iso CDVD 0.7.0" plugin I know it fixed my problem.

No,vtlb miss is NOT directly related to anything. As I already said,it just means PCSX2 crashed and that could be for a million reasons,from bad settings to incompatibility with some games.

Well for me the majority of the time it happens is when it tries to load up a new section of a game whilst using the drive plugins.
I think the iso plugin is more stable not to mention it's quicker so it creates less problems for the emulator.

I was trying to run FFX, I was using GSdx 0.1.14 SSE3 with a T7100 Intel (yes, it supports SSE3), with SPU2-X sound plugin, with Linuz ISO CDVD 0.8.0. My GPU is an Nvidia Quadro NVS 140M, I have 3 GB RAM, and I run Vista. I seem to have configured everything properly, and if i try to un the ISO directly without going through the BIOS process, it crashes right after HDD enable = 0. Can that help?

Nope.
Would need stack traces etc.. just saying it happened here and there is too vague for that sort of error as has been already explained a million times by krakatos, bositman, rama and several others.
