I use alcohol 52% (freeware edition)to rip most cds/dvds.

use the EFP ISO plug-in

go to "Get from disk" button..then the upper box is your source drive e.g. (D:\)
the 2nd box says [Put an Image File Here].....just name that file you wish to create the iso.
then press "Make File".

then the your ISO is created..

Then again, if you want to literally make it a file with an ".iso" extension, use DVD decrypter instead.

Click on the pretty blue link in zero29's post and you'll hopefully understand. It's a program to create and burn ISO files, the 'create' part is the one you're interested in.

DVD Decrypter is illegal under US law, specifically the DMCA sections regarding content protection circumvention tools. It's why development has stalled for years, the project was basically abandoned
ImgBurn was born from DVD Decrypter's ashes. People removed the code that offended US law (CSS decryption stuff) and continue to update and support ImgBurn as a stand-alone application. For anything that doesn't require ripping DVD video, ImgBurn is superior on all fronts.

Not sayin' I agree with that particular portion of US law, but I'd like to let people make an informed decision.
