You are using too old omniconvert version and if I remember correctly,this can case problems
http://www.herrvillain.com/omniconvert/

While using the latest version,the codes don't look that way

What are you talking about? the version i have is 1.0.3, even the link you gave me the most up to date version is the one i already have.
EDIT: Are you referring to the 1.1.0 that you created\? by chance i clicked on your cheats decrypter link. so i am correct in assuming you use this with pcsx2 cheat editor when converting codes?

Sorry,I thought it was the latest version in the link I give you.
The omniconvert in my thread is the latest version but the program is not mine and yes,I used it along with my multi converter(which is actually running omniconvert as a hidden process to do the decrypting and converting to pnach)

I use version 1.1.1 of Omniconvert.

REOhmyutbreak is pretty picky with codes btw. Even with properly converted codes it can give the TLB errors. Though I'm not sure which cheat in particular was causing it for me, didn't have time to test it.

No luck i'm afraid, it's the PAL version i have if that might be the issue?

Try these. (It's possible these might not work on PCSX2 1.2.1, requiring you to use the newer dev versions due to being D and E-types.)

Code:
// Infinite & Max HP
patch=1,EE,E0023880,extended,00660794
patch=1,EE,206607A0,extended,2403270F
patch=1,EE,206607A8,extended,AE030544

// Infinite Ammo
patch=1,EE,D05B1FC8,extended,00001E3C
patch=1,EE,205B1FDC,extended,00000000

// Infinite Item Usage
patch=1,EE,E0031E3C,extended,005B2148
patch=1,EE,205B215C,extended,00000000
patch=1,EE,205B2170,extended,00000000
