Additionally, could you also try using software mode at the specific scene toggling insert key ? that would allows us to verify whether it is a Mipmap issue.

I'll make sure to not use that particular hack next time I do a bug report and yes, this bug is independent of that hack. Also, while the MTVU hack isn't enabled by default in the Safest preset, it isn't grayed out like all the other options, not sure if that's intentional or a GUI bug.

I usually play stuff in HW mode; when I spot something off, I toggle SW mode with F9 to confirm that the bug only happens in HW mode.

Mtvu is a special hack. For some games it doesn't help but slow you down. For some games mtvu will create a hang or freeze. On some systems it slows you down independent in the game. On some systems it helps for almost all games or does at worst nothing bad. It is a very effective unpredictable weapon. Therefore it is not controlled by the slider.

The upload probably expired. Not sure if I should go make another one because it was made clear that this bug is simply due to mip mapping not being implemented at all.

please try pressing "Insert key" and check whether the certain bug reappears on software mode , Insert toggles between Mip-Mapping on software mode, this would help confirming whether the issue is based on mip-mapping.

Yes, I can confirm. On software mode, toggling mipmapping with the Insert key makes the bug appear and disappear.
