Read first: @How to submit your testing results
Yeah that could work, bout being tied to the little, medium, much system would be advantageous if we could use hours as an alternative

PC Specs:

Build Description: PCSX2 1.4.0 - Windows
If other please specify: N/A

BIOS Used: Europe v01.70(27/02/2003) Console

Preset (off/1-6): Off

EE/VU Clamping Modes: Normal/Normal
EE/VU Round Modes: Negative/Chop

Speedhacks Used: N/A

Gamefixes Used: SkipMpeg, EE Timing Fix

Automatic Gamefixes: Enabled
Cheats: Disabled
Widescreen Patches: Disabled

GSdx Settings: OpenGL Hardware, 4x Native, Blending Unit Accuracy=High, Texture Filtering=Bilinear(Forced)

GSdx HW Hacks: Half-Pixel Offset=Special-Texture

Amount of testing done in hours: estimated 6-7 hours
Completed game (no/yes): no, about half-way

Seems to play fine, FPS sits around 50 FPS with no speedhacks.
Using GSdx Software renderer is noticeably slower at around 40 FPS.

Slight SPS from some characters, but nothing game breaking.

Game freezes on menu video unless the SkipMPEG fix is used.
Graphics garbled unless you use the EE Timing Fix.
Broken AI behaviour unless EE Round Mode is set to Negative.

New example template suggestion:

Ordered the Emulation Settings
Added Preset setting
Added Automatic Gamefixes/Cheats/Widescreen Patches

Renewed testing time/completion.
Added Performance section.
Minor cleanups
Looks good to me.
Does the necessity of using HW hacks disqualify the game from Perfect status?
