Xenosaga 1 cheat help

I was replaying the game for nostalgia, and figured some cheats wouldn't hurt.
Before I get to the question, I'll put what relevant info I can.

I used Almar's guide for converting, used the correct conversion types (CodeBreaker V1 to Raw/Unencrypted), and checked on here.
Somehow, the cheats just aren't working still. It's driving me nuts.

The text file is located in the PCSX2 Cheats folder under Documents, and it is a .pnach file extension (PNACH file type), not .txt. Using the NTSC version. The file is set as 6D1276AB.pnach, the Game CRC I got from the "show console" command.
It is finding and loading the cheats, but there still seems to be an error and I'm honestly not sure what's causing it to happen.

Is there a specific way to do that? Just only put one cheat in the .pnach at a time or?
This is the first time I've used cheats on here, and only the second time I've run a ROM through the PCSX2.
Aside from what the guide showed, I have no idea what I'm doing.

Nope, that straight up crashes the game as soon as I get control of Shion after the cutscene.
If I disable the TP cheat, it still crashes the game.
