Xenosaga episode one error code

Im having trouble with playing xenosaga. It plays well at first but when i get to a certain point in the beginning cutscene i get this:
CDVD READ ERROR, sector = 0x003f1801
CDVD READ ERROR, sector = 0x003f1803
I tryed plug-in: GSdx 5875 (MSVC 18.00, SSE2) 0.1.16 [gsdx32-sse2-r5875]
and SSE41 and SSE3 same error codes
I would like to know how to fix this issue.
Dell Inspiron 15
Graphics: Intel® Core™ i5 CPU M480 @ 2.67Ghz 2.66Ghz
RAM: 8 GB
OS: Windows 7 Ultimate 64-bit

Plugins Currently using:
GS: GSdx 5875 (MSVC 18.00, SSE41) 0.1.16 [gsdx32-sse2-r5875]
PAD: LilyPad svn 0.11.0 [lilypad-r5875]
SPU2: SPU2-X r5830 2.0.0 [spu2-x-r5875]
CDVD: cdvdGigahertz (r5822) 0.8.0

Also another error code that pops-up at random times is:
[wx] Failed to load shared library 'C:\Program Files (x86)\PCSX2 1.2.1\Plugins\gsdx32-avx-r5875.dll' (error -1073741795: unknown error c000001d)
Path: C:\Program Files (x86)\PCSX2 1.2.1\Plugins\gsdx32-avx-r5875.dll
File is not a valid dynamic library.
Some kinda plugin failure: C:\Program Files (x86)\PCSX2 1.2.1\Plugins\gsdx32-avx-r5875.dll

Im pretty confused about this.

Sounds like your disc is scratched

The plugin failure is normal, just means your CPU don't support those extensions.

Anyway, try make an ISO from your disc.

The try to rerip it from your DVD. Xenosaga is a dual layer disc. Use Imgburn to make the iso.

Remake your iso with imgburn

so you want me to re-create the same file? you want me to write it to a disc? im just confused on where to extract it from
im sorry if its such a simple thing for you and im like not getting it :/

1. Put your xenosaga disc in your dvd drive.

2.launch imgburn

3. Select create iso from disc

4 Hit start.

I have the iso file already its a rom file >.>
Refering to the other commment before.... rerip meaning put it on a disc then extracting it back from it?
