How about a template, like those that show up when making issues in googlecode. Sounds better than an annoying popup, and quickly editable to add the information.

I could look into a pop-up plugin for the forum when a user creates a new thread to prompt them to post that info

this could work... pop ups arent annoying its just 1 click away if your familiar with it.
