Are you sure that this is an iso used directly by pcsx2? not that you mount the disk by using some virtual drive program and use the gigaherz-plugin to load from this virtual drive.

Does any other game work?

I had the exact same problem when I use custom internal resolution(set to 1280x720 and even at the game native resolution)

Either use native or scaling if you want better quality...custom is useful only in rare cases.
