pnach and how to use it

I was playing persona 4 and i made pnach file using notepad then convert it to pnach file.
i put it like the following

how do i run the pnach file?
how do you find the address and stuffs?
i've tried the thread http://forums.pcsx2.net/Thread-How-pnach-files-work but i dont really understand. if it is complicated, tell it to me.

and in persona 4, i tried to find the address for the hp using cheat engine and i can't find it.
