you are using too much hacks and fixes,
try disable ALL speedhacks and ONLY enable the tri-ace's gamefix.

Try using the stable PCSX2 0.9.8 and if it still works please post the content of emulog.txt AFTER you get the problem.

SOLUTION!

Mount .iso with the daemon tool and configure cdvdgigaherz plugin

"Luckily" 0.98 prevents easily changing the NTSC and PAL defaults, which should not be changed under risk of messing badly the game's (and emulator's) timings.

PS: for some reason I think people think it is related with framerate instead "refresh rate". It's already hard to reach 60 FPS (from PC standpoint) in NTSC... yet harder would be to reach (expected) 80! better not commenting that 90 for PAL.
