what code source is that??? CB? GSv3-4?? GS 5+?? or AR??
and are you sure the CRC ID is correct?
best idea to check the game CRC using 0.9.6's console window to reveal the game CRC.
PCXX2 0.9.8 will too show CRC of the game
Whats you game CRC. Make sure the code is of the region as your game or it won't work.

Well, Your files was totally wrong. Read the Guide for some info:-
1)Get the AR-MAX / GameShark Codes and you need to convert them to Raw Mode(I guess GS works fine without changing). Convert it with Omni-Convert.
2)Then you need the CRC.
3)Put the Codes in this format

patch=1,EE,xxxxxxx,Extended,yyyyyyyy
And save the files a
CRC.pnach
And run the game witht he cheats enable.

Mine one was the converted one. You put the AR-MAX Codes directly without formatting and converting.
