I have a possible fix.
Change the first 0 to a 2 before you convert the address part of the code (using Omniconvert I assume)
For example, 003A9448 becomes 203A9448.

I tried just changing the 0 to a 2 like you mentioned before, but I get the same result. I thought these codes could be loaded straight as .pnach files with ps2rd and FreeMcboot. I think there has only been a few games that only work by switching the 0 to a 2 and that has worked in the past.
For some reason this game will not work.
That sucks. :/
Sorry, but I don't really have any other suggestions. Guess you'll have to 4:3 for now.
I really hope you can get a decent PC which can play Outbreak someday. I do and I play via the fan server and it's fantastic.
Speaking of Outbreak, can anyone please try making two patches for me?
The first is one that can turn off the blue filter when you check the inventory.
I find it rather ugly and would prefer a clear view of the environment.

It'd also be cool if the inventory HUD could be narrowed, but I'm not sure if that's possible.
The second one is a patch that would make you run by default and make it so that you walk by holding the run button.
