I've been tinkering for a few days (the entire Midwest of the United States is frozen over, so I've had much time to tinker), and I believe I've compiled the definitive Front Mission 5: Scars of the War configuration.

None of the following work is my own; I just took the various elements and stuck them together into one package, and I would like to share the steps taken.

What you will need:

Front Mission 5: Scars of the War. The MD5 you want is: b5eadad7456e3f2eb27ef4a79be63377

Steps:

First, verify your FM5 MD5-- because I didn't, and I spent forever trying to figure out why my shiny, new Ultimate Hits edition did not work.

Second, open up ImageDiff with the ImageDiffGUI.exe.
Under the "Patch Image" tab, begin by Browsing for the Patch. The patch was frontmission.info's "FRONT_MISSION5_BETA_PATCH.ISO" from within the patch4-complete folder.
Under Foreign Image, Browse for your FM5 iso.
Under New Image, Browse for a destination and create a name for the patched ISO.
Click "Go!" and wait about five minutes. When the process is done, navigate to your new file and rename it to include the .iso extension.

Third, open the "ps2_patch_engine.exe" from within the ps2_patch_engine_103 folder.
After unpacking 99skull's patch, click and drag "2615F542.pnach" into the PS2 Patch Engine's GUI.
Then, Browse and select your English patched FM5 iso.
Click "Patch" and select an output destination and name. When the process is done, fire up PCSX2 version 1.0.0.

PCSX2 1.0.0 Settings:

You'll note that there is an insane amount of artifacting, stuttering and distortion upon booting up the doubly patched game. To remedy this, follow along:

General Plugin Settings:
Have Automatic Gamefixes enabled.
Set the Aspect Ratio to "Widescreen (16:9)".
Deselect Wait for Vsync on refresh.
Disable all speedhacks. I don't think any would hurt significantly, but what I wanted was a bare-bones configuration to play the game at optimum emulation settings.

GSDX10 Settings:

Renderer: Direct3D11 (HW)
Interlacing: Auto
D3D Internal Resolution: 2-6x Native (The higher you go, the more you will see text artifacts)
Texture Filtering should be checked.
Check Enable HW Hacks.
Within HW Hacks Configuration, set MSAA to as much as you can handle. Set Skipdraw to 9. 5 causes artifacts in battle and 10 causes artifacts in cutscenes, so after a lot of testing, 9 seems to be the balance. Finally, enable Half-pixel Offset to get rid of the blurry haze that envelops background objects.


At the end of all of that, Front Mission 5: Scars of the War should emulate with just minor text artifacts, but it will do so in 16:10, in English and with a pumped up resolution.
