Simple, you have a xyz.pnach.txt file instead just xyz.pnach as clear at the append abd everybody above pointed out. Solution: remove the .txt

Have you actually enabled cheats in the menu of pcsx2? (System>Enable Cheats)

Does it happen only with this game? or with all games?
1. If you use a CDVD plugin - Don't. Use CDVD -> Iso Selector -> Browse ...
2. Make sure that the game indeed loads regardless of cheats.
3. If you use full boot, try also fast boot.
4. Also try a more recent version of PCSX2.
Explanation:
This message is displayed when PCSX2 doesn't know the CRC of the game you're loading (yes, I did see at your log that a CRC exists, but still for some reason when it tries to load the cheats it doesn't know what CRC to use).
It has nothing to do with incorrect pnach file etc.
It's typically displayed when booting the BIOS without a DVD (hence [2] above to make sure that the game starts and the disk is inserted).
It's also possible that when doing a full boot, it starts without a CRC but later it gets a CRC from the DVD. So there could be a bug - which is why I asked to also try fast boot.
It's also possible that if there's a bug, it might fixed in a more recent version than yours, hence [4].
