Currently using 1.5.0 dev 2232, but same issue occurs on 1.4 official release. I've tried disabling 60fps hack and widescreen hack and get the same problem. From the research I've done that seemed to be the common solution for this kind of problem but doesn't seem to help me.
I just got a 1080 ti today. Played through MGS3 on an RX480 using dx11 and only had this issue once toward the very end of the game. Replaying it using Opengl with my new 1080ti and it's crashing a lot more and typically in the same spot. Tried changing back to dx11 and that didn't help either. Disabled speed hacks and even played in software mode and i'm getting these crashes still.

Strange, I never had this issue during my runs of MGS3.

You can try re-ripping your game maybe if it still occurs with all cheats off like you mentioned. Make sure you use imgburn for it.

I just remembered I never restarted my computer after installing the new driver. I just immediately went to redeem the game that came with it and started playing pcsx2 while it was downloading. Maybe that would have something to do with it.

Check your PCSX2 plugins, it looks like you're using older ones.

Hmm might be. I got the latest revision of pcsx2 but I kept all the same plugins

TLB miss also occur on invalid cheats and patches..

just tried using new plugins and still getting TLB Miss error.

use 2-directsound

already am with interpolation at 4 catmull-rom

Here's my GSdx settings, although I'm not sure they matter given that this issue still occurs on software rendering as well.
