Too bad if you can not use direcxt.10 mode, many case will help you to gain more fps in games and can fix from compatibility issue. Perhaps about blank screen problem because you already use speedhacks too much. Btw you can try with vyscreen On at gsdx plugin.

v sync. has nothing to do with those and will lower his fps... except if u have a monster pc able to play any game at high res also u shouldnt use v sync. u will not see any big difference also...
v sync's work is to reduce the tearing effect that happens some times in (mostly) pc games when they run faster than your screen output and what it does is keep frames to the max your screen Hrz support. in pcsx2 works different ofc. but its not recommended for use in slow/medium pc's.

edit: u can try using the latest svn builds and look if it happens there also. (ofc u will not get d3d 10 without the service pack 2 and the rest windows updates)
