Curious what it's reporting then. maybe some additional hidden sony stuff? unless it's not polling the right thing for power sometimes, or just a bug somewhere. or possibly the timings being slightly off depending on what bluetooth dongle people are using?

Yes macros can do that if you use the pause function

If you copy the log from within the application it will format it for you for forum use.
Really? Before I copied the log i wanted to recreate the problem, but obviously this time it almost worked just as it should've, but I managed to recreate it With shadow of mordor when it was launched through steam, but i didn't try any more games after that. I have my desktop profile where i use the sticks as mouse, then i open steam and it switches to default, then i launch shadow of mordor and it switches back to desktop again.

SoM switched to the [Default] profile as it should, it looks like you immediately quit or minimized though which is why it went back to desktop:

Code:
19.10.2014 19:36:05> TRACE: Active application changed to E:\Steam\SteamApps\common\ShadowOfMordor\x64\ShadowOfMordor.exe
Default profile [Default] being used.
19.10.2014 19:36:13> TRACE: Active application changed to C:\Windows\explorer.exe
Custom profile Desktop being used.

When I do a first pair with my DS4, it pairs successfully, DS4 Tool and this tool both detect the controller just fine. Everything works as expected. Now here's where it gets annoying. I disconnect my controller, come back at a later time, turn on my controller. It successfully reconnects to my computer. I can see the "Wireless Controller" device and it functions as a normal DirectInput controller. However, neither tool can see my controller as if it was connected. DS4 tool "sees" my controller kind of, but the LED settings don't kick in, touchpad doesn't work, and the controller doesn't function as an XInput controller, then the ScpServer freezes for a few seconds, unfreezes, and repeats until I just force close. Then I run this tool and it doesn't even show up as it's connected. But I can assure you I go to my devices, and it's connected and like I said, functions as a normal controller.

The only way around this is I have to remove my controller and re-pair it everytime in order for these programs to detect my controller. I'm wondering if anyone else has had this issue or if anyone knows how to fix this. Here's a link to my adapter.
http://www.amazon.com/gp/product/B007Q45...UTF8&psc=1

Which I don't think it is, using the drivers that came with it, and my DS4 pairs with it just fine and I can disconnect/reconnect whenever. I'm running Windows 8.1 if it matters, I've tried with both exclusive and non exclusive mode. Any help is appreciated, I'm really desperate.

Here's a screenshot of the log when I connect my controller after a first time pair.

You switched between steam and explorer a couple times but it looks like the profile changed correctly with the switches

some more debug output here with the new .dll

Code:
A first chance exception of type 'System.IndexOutOfRangeException' occurred in DS4Library.dll
A first chance exception of type 'System.IndexOutOfRangeException' occurred in DS4Library.dll

seems to be catching the exception gracefully, controller still working. was not using the controller at the time, so I don't know if there was any performance side effects yet.

Just a fyi, try to avoid spoiler tags on these, it hides it from search engines as well and its nice to have the stuff searchable incase another user has the same error.

Your BT adapter is compatible but it appears that a driver or other software is causing issues with the packet reporting, try uninstalling the adapter completely drivers and all and removing any software that you installed for it. Then plug back in and let Windows reinstall the basic drivers.

Also try other ports.

You also mention you have other tools, make sure none are running and that you do not have joy2key or joy2mouse or anything like that installed.
