Running OSX Lion. Installed XQuartz and the Nvidia CG.

Just a Wild guess : did you try to chmod your pcsx2 dir ?
CPU : AMD Ryzen 7 3800X
Mobo : Asus PRIME B450-PLUS
GPU : NVIDIA GeForce RTX 3070

Nope. What is the command I would need to type? It does seem like it's a permissions issue.

chmod -R 665 /Applications/pcsx2.app/Contents/MacOS/pcsx2
this is just an idea I got from basics in Unix shell since I don't own a Mac

Are you logged in as an administrator with all permissions? If not, make sure you are, then delete pcsx2.app and reinstall (don't worry about reinstalling xquartz or cg, if those are installed, you should have nothing to worry about).

I thought the account I have is the administrator. Under "Users & Groups" it says my name and then "Admin". The only other user is guest.
How do I check is this is the actual administrator account or merely an account with admin privileges?

Right click pcsx2.app > get info > sharing and permissions drop-down box > make sure you have "read & write" enabled.
Do the same for xquartz.
Once you do this, delete and then reinstall pcsx2, see if that works.
