It's complicated for PCSX2 is not a game, to make it work as you want you need to copy the iso file itself as a shortcut to desktop and associate .iso with PCSX2. that will work but be aware all and every ISO file will try to open PCSX2 if clicked.
PS: make sure you have "Iso" and not plugin set under the CDVD tab.
If you do as said above, ISO files will get a nice PCSX2 icon, you can change it for the desktop game's shortcut if you have a sample.
To change the application to start that kind of file, right click any .iso file (the game's disk image itself does the trick), click the button "properties" and
at "Open with" click change and navigate to your active PCSX2 directory and click the executable.
BUT, having a shortcut to PCSX2 on the desktop and starting games from the gui using Iso (and not plugin and having to mount isos in virtual drivers) is cleaner and very easy.
