your CPU might be downclocking, check your processor condition with a app like core temp and show the clock speed and temperature readings while using pcsx2 to play the game. Never mind.
it's something weird if, disabling that gave you a speed boost. I'm not certain of the reasons anyway, glad you got it sorted out.

Currently I can not open pcsx2 but I think mtgs is one of the recommended hacks.
Additionally there is this synchronize gs option in the gs tabpage of emulation settings. And behind that option there is a huge "warning: extremely slow!!!". And that option you should never tick.

yeah you are right. I thought MTGS is one of the recommended speedhacks. But obviously it is multithreaded without an option to disable it.
Probably synchronized MTGS more or less disables multithreading due to synchronization overhead.

MTVU is a speedhack, MTGS was an option back in the day to put the GS on a separate thread, before PCSX2 was stable over multiple cores
