Not sure if it will do something different, but you can try converting pnach to HEX code using PNACH converter then apply it to your ISO.

PCSX2 doesn't have an internal FPS counter (ingame FPS), situation that makes very difficult to find 60 FPS codes for games, but lucky me I still found it for Metal Gear 3 Subsistence and of course you can see a HUGE difference!
Code: gametitle=Metal Gear Solid 3 - Subsistence Disc 1 (Limited Edition, SLUS-21359) Code: gametitle=Metal Gear Solid 3 - Subsistence Disc 1 (Limited Edition, SLUS-21359) We all know that this game needs a powerful CPU to achieve full speed at 30FPS, so with the 60 FPS code enabled we need a MUCH MORE powerful CPU. According to some testing, the EE needs at least 20% overclock to work perfectly or else the emulated PS2 can't reach 60 FPS. The main speed problem with the game are the cutscenes, that's because the game runs at 60 FPS ONLY while these are playing. That's why I included the 30 FPS cheat (gameplay original values), with this code enabled the cutscenes are a lot less CPU demanding and the game is more enjoyable with not so powerful hardware.
